The agency notes that the disease has been growing at roughly a 3% annual rate from 2014 through 2022, with the sharpest uptick seen in advanced‑stage diagnoses.
Rising Incidence and Advanced‑Stage Diagnoses
According to ACS data, the steady increase in case numbers is largely driven by more men undergoing screening tests. Yet, the organization cautions that many men are still not being screened early enough. “We’re seeing two things happen at once: more men are being diagnosed with prostate cancer, and a growing share of those cases are happening at a more advanced stage,” an ACS representative said.
Prostate cancer remains a common disease, affecting roughly one in eight men over a lifetime. When detected before it spreads beyond the prostate, the five‑year relative survival rate exceeds 99%. However, the disparity in incidence is stark: Black men experience nearly a 70% higher rate of prostate cancer compared with white men. Screening guidelines in the United States emphasize shared decision‑making between a patient and clinician. Men of average risk are advised to consider screening at age 50 if they have a life expectancy of at least ten years. Those at higher risk should begin discussions at age 45, and men with a strong family history or multiple affected relatives may start as early as age 40.
Dr. S. Adam Ramin, a board‑certified urologist and medical director of Urology Cancer Specialists in Los Angeles, highlighted the profound impact of early detection. “The benefits of detecting prostate cancer at an earlier, localized stage are profound,” he told Healthline. Early‑stage disease offers a broader array of treatment options, including active surveillance, surgery, and various radiation therapies, many of which are minimally invasive. Localized cancer carries an almost 100% five‑year survival rate, and less aggressive treatment can preserve urinary and sexual function, improving quality of life.
The prostate‑specific antigen (PSA) blood test is the primary screening tool. Dr. Ramin explained that while an elevated PSA does not automatically indicate cancer—benign prostatic hyperplasia and prostatitis can also raise levels—it serves as a critical first‑alert signal. Abnormal results may be followed by a digital rectal exam, repeat PSA testing, imaging such as MRI, or a prostate biopsy to clarify the diagnosis.
Risk Factors and Who Should Consider Testing
Age remains the strongest risk factor, with six in ten cases diagnosed in men over 65. Men of African descent, including African Americans and Caribbean individuals of African ancestry, face higher incidence rates. Obesity is linked to more aggressive disease, and a family history—having a father or brother with prostate cancer—more than doubles a man’s risk.
Dr. Ramin emphasized the broader public health message: “One of the most critical messages that Prostate Cancer Awareness Month or celebrities can convey when sharing their cancer diagnoses is the importance of early detection. This knowledge empowers individuals to take control of their health and seek early detection.” As the disease continues to rise, the ACS and medical experts stress that timely conversation with healthcare professionals can save lives and reduce the burden of advanced disease.
