The experimental agent, aglatimagene besadenovec, is designed to be delivered directly into the tumour, where it kills cancer cells and triggers an immune response against residual disease.

Trial design and key findings

The study enrolled patients with intermediate or high‑risk localized prostate cancer—populations for whom surgery and radiation are established curative options but where recurrence remains a concern. Participants were randomly assigned to receive either radiotherapy alone (the control arm) or radiotherapy combined with aglatimagene besadenovec (the experimental arm). The trial’s primary focus was disease‑free survival, a standard metric indicating the length of time after treatment during which a patient shows no signs of cancer.

Results showed a benefit in disease‑free survival for the combination strategy. In an exploratory analysis of a subgroup of patients, 80 % of those who received the experimental therapy had no detectable tumour cells in prostate biopsies taken two years after treatment, compared with 63 % in the control group. While these figures suggest a favorable trend, the biopsy outcome was not the trial’s primary endpoint and therefore requires cautious interpretation.

Context within prostate‑cancer treatment

Prostate cancer management has evolved from a straightforward diagnosis‑and‑treat model to a nuanced approach that weighs tumour aggressiveness, recurrence risk, and quality‑of‑life considerations. Standard modalities—surgery, radiotherapy, and hormone therapy—address the disease through distinct mechanisms. Radiation controls local tumour growth, hormonal therapy reduces androgen‑driven proliferation, and immunotherapy, as explored in this trial, adds a potential third line of defence by mobilising the body’s immune system.

Historically, immune‑checkpoint inhibitors have shown limited efficacy in localized prostate cancer, restricting immunotherapy to advanced or metastatic settings. The viral vector used in aglatimagene besadenovec differs from checkpoint blockade by delivering a therapeutic payload directly into tumour tissue, thereby killing cancer cells and exposing tumour antigens to immune cells.

Dr. Ashwin Giridhar, senior consultant and lead of urology & uro‑oncology at HCG Cancer Hospitals in Bengaluru, highlighted the significance of investigating immunotherapy at an early disease stage. He noted that even when imaging shows no visible tumour, microscopic disease may persist, and early immune activation could help eradicate these hidden cells. However, he cautioned that a promising mechanism does not guarantee clinical success; further trials and longer follow‑up are needed to confirm whether the observed benefits translate into durable, real‑world outcomes.

Importantly, the trial’s exploratory biopsy analysis, while encouraging, does not replace the need for robust primary‑endpoint data. Dr. Giridhar emphasized that clinicians must continue to base treatment decisions on established factors such as PSA levels, Gleason scores, imaging findings, patient age, overall health, and personal preferences. Surgery, radiotherapy, and hormonal therapy remain essential components of individualized care, and any new therapy must demonstrate clear, reproducible advantages before altering standard practice.

The study also underscores a broader shift in oncology toward combination strategies. By layering different therapeutic mechanisms—local control via radiation, systemic hormonal suppression, and immune activation—physicians hope to improve cure rates while minimizing side effects. The ultimate goal, according to Dr. Giridhar, is not to replace effective existing treatments but to enhance them, making them work better for each patient.

Future research directions include identifying biomarkers that predict which patients are most likely to benefit from immunotherapy, determining the durability of the immune response, and assessing long‑term survival and quality‑of‑life outcomes. If these questions can be answered, viral immunotherapy could become a valuable addition to the curative armamentarium for localized prostate cancer.

For now, the Phase 3 trial provides a positive signal that warrants further investigation. The medical community will be watching closely as additional data emerge, hoping that the integration of immunotherapy into early‑stage prostate cancer treatment may eventually improve prognosis without compromising patients’ quality of life.

A Phase 3 double‑blind trial published in The Lancet shows that adding the viral immunotherapy aglatimagene besadenovec to radiotherapy improves disease‑free survival for men with localized high‑risk prostate cancer.

[299
3115,3114,3117,3113,3116
post-2811","post","type-post","status-publish","format-standard","has-post-thumbnail","hentry","category-news","tag-clinical-trial","tag-immunotherapy","tag-oncology","tag-prostate-cancer","tag-radiotherapy","entry
["GET
