Midlife Vascular Health Linked to Up to 13 Extra Dementia-Free Years

New research published in Neurology Open Access indicates that individuals who maintain optimal vascular health during middle age can expect to live substantially longer without dementia. The study examined more than 12,000 participants and found that the absence of three key risk factors—high blood pressure, diabetes and smoking—between ages 48 and 68 was associated with an average gain of nearly 13 dementia‑free years.

Study design and key findings

The investigators defined good vascular health as having none of the three risk factors during the specified age window. Participants were grouped according to how many of these factors they exhibited at age 55, and the length of dementia‑free life was calculated for each group. Those with no risk factors lived, on average, 30 years without dementia. By contrast, individuals with one risk factor saw this period drop to 26 years, two risk factors reduced it to 21 years, and having all three risk factors shortened the dementia‑free span to just over 17 years.

These differences translate to roughly a 13‑year advantage for people who avoid hypertension, diabetes and smoking compared with those who have all three conditions. The study also reported that participants with three midlife risk factors faced almost three times the hazard of developing dementia relative to risk‑free peers.

Implications for brain health and public policy

Senior investigator Josef Coresh emphasized the practical relevance of the findings, stating, “Our findings argue that people need to actively avert these factors in midlife as a strategy for preserving brain health for more than a decade.” He added, “Hopefully, these results will encourage people to stop smoking and watch their vascular health closely from age 48 on.” The research arrives at a time when dementia is one of the fastest‑growing health burdens worldwide, with the World Health Organization reporting 10 million new cases each year and projecting more than 150 million cases by 2050.

Because the identified risk factors are modifiable through lifestyle changes, medical interventions and public health initiatives, the study supports a preventive approach to cognitive decline. Strategies such as regular blood pressure monitoring, diabetes screening and smoking cessation programs could substantially reduce the future dementia burden.

Sex differences observed

The analysis also uncovered gender‑specific trends. Women generally enjoyed longer dementia‑free periods than men across all risk categories. For example, among participants with three risk factors, women averaged 18 dementia‑free years compared with 16 years for men. This suggests that while vascular health benefits both sexes, women may experience a slightly greater protective effect.

Overall, the findings reinforce the importance of midlife health management as a lever for extending cognitive vitality into older age. By targeting hypertension, diabetes and smoking, individuals and health systems alike can work toward adding valuable, dementia‑free years to the population’s lifespan.
